In accordance with Part 173 of the General Business Law, the Secretary of State requires that no services shall be rendered on behalf of a client until the following Advance Statement of Services and Charges is delivered to and agreed upon by the client.

It is understood that Joe O’Brien Investigations, Inc. (the agency) shall perform the above-described services according to its own methods, in accordance with Federal, state and local laws and within the scope of authorization and operation as requested by client.

The agency’s standard investigative rate for this type of case is $---------- per hour.

Out-of-pocket expenses, associated with this investigation, are the responsibility of the client and will be deducted from the retainer. Payment in full is due upon completion of assignment, if the cap, authorized by the client, is greater than the amount of the retainer. Outstanding balances, carried beyond 30 days will be charged interest of 1.5% compounded monthly. Client further agrees to pay all costs of collection, plus reasonable attorney fees. Client may terminate the investigation at any time and the amount of the retainer not earned out will be refunded to client. It is understood that the agency does not guarantee success or desired results in this investigation. A retainer of $-----is required before work is commenced.
